| Abstract: | Type 2 diabetes mellitus (DM2) and nonalcoholic fatty liver disease (NAFLD) have a high prevalence worldwide and frequently coexist, so that DM2 is an important risk factor for the more rapid progression of NAFLD, in turn NAFLD is an independent risk factor for the development of DM2, so knowing the bidirectional pathophysiology between these two clinical entities allows having a broader knowledge of the subject to improve the management of this type of patients. Therefore, the aim of this review was to describe the pathophysiological interrelationship between DM2 and NASH. To this end, a search was carried out in PubMed and 714 articles were found, with the subsequent application of inclusion and exclusion criteria. The articles included in this bibliographic review were 33, including systematic and literature reviews, meta-analyses, clinical trials and observational studies corresponding to the last 10 years. It was concluded that the mechanisms described, such as insulin resistance in different tissues and the role of lipotoxicity in the previous mechanism, have updated evidence in favor of them, despite the low degree of evidence. On the other hand, mechanisms such as the role of lipotoxicity in NASH progression and the role of cytokines in insulin resistance still need the contribution of more studies to find firm support for these theories in humans. |
| Description: | La diabetes mellitus tipo 2 (DM2) y la enfermedad por hígado graso no alcohólico (EHGNA) tienen una alta prevalencia a nivel mundial y coexisten con frecuencia, de tal manera que la DM2 constituye un factor de riesgo importante para la progresión más rápida de la EHGNA, a su vez la EHGNA es un factor de riesgo independiente para el desarrollo de la DM2, por lo que conocer la fisiopatología bidireccional entre estas dos entidades clínicas permite tener un conocimiento más amplio del tema para mejorar el manejo de este tipo de pacientes. Por tanto, el objetivo de esta revisión fue describir la interrelación fisiopatológica que mantiene la DM2 con la EHGNA. Para ello se realizó una búsqueda en PubMed encontrándose 714 artículos, con la posterior aplicación de criterios de inclusión y exclusión, los artículos incluidos en esta revisión bibliográfica fueron 33 incluyendo revisiones sistemáticas y de la literatura, metaanálisis, ensayos clínicos y estudios observacionales correspondientes a los últimos 10 años. Se concluyó que los mecanismos descritos como la resistencia a la insulina en los diferentes tejidos y el rol de la lipotoxicidad en el anterior mecanismo, tiene evidencia actualizada y a favor de los mismos, a pesar del bajo grado de evidencia. Por otro lado, mecanismos como el papel de la lipotoxicidad en la progresión de EHNA y el papel de las citoquinas en la resistencia a la insulina aún necesita el aporte de más estudios para encontrar un respaldo firme de estas teorías en humanos. |
