Predictores de via aérea difícil: validación de los tests de Mallampaty, Patil Aldreti, distancia esternomentoniana y apertura bucal frente a la laringoscopía directa para intubación orotraqueal Hospital José Carrasco Arteaga, Cuenca 2009

Abstract

Objective. Validating the proofs of: Modified Mallampati, Thyromental Distance (TMD), Sternomental Distance (SMD) and Buccal Opening, separately and combined, in front of the Laryngoscopy, like predictors of difficult air route.

Materials and Method. Were evaluated 250 patients with ages between 16 and 72 years, of both sexes, ASA I and II, submitted to elective low surgery general anesthesia whom in the pre-operative I accomplish to them his measurement of: Modified Mallampati, Buccal Opening, Thyromental Distance and Sternomental Distance, right after induction and with the patient the direct laryngoscopy had total success in a diagram made suitable for the intubation, considering oneself grade III and IV according to Cormack's-Lehane's method like intubation difficult orotraqueal.

Results. The incidence of difficult intubation went from 6 %. Sensibility, specificity, predictive positive value, predictive negative value of the 4 proofs was Modified Mallampati ( 73,33 %, 85,11 %, 23,91 %, 98,04 % ) AB ( 40 %, 98,72 %, 66,67 %, 96,27 % ) DTM ( 53,33 %, 99,57 %, 88,89 %, 97,10 % ) DEM ( 60 %, 99,15 %, 81,82 %, 97,49 % ). The following was obtained when combining the four proofs: Sensibility: 90 %, Specificity: 97,50 %, Predictive Positive Value: 60 %, Predictive Negative Value: 99.57 %.

Conclusion. The proofs here described have not much value like predictors of difficult air route But it is necessary to score than if the easy orotraqueal predicts an intubation itself, this will really be easy in a loud percentage
