dc.descriptionEn el peri-operatorio es importante disponer de un método que permita discernir si la punta del catéter venoso central está en una posición ideal o no. El ultrasonido por contraste de micro-burbujas lo permite, sin embargo, la investigación está recién en auge en pediatría. Se realizó un estudio de test diagnóstico con el objeto de determinar la validez del método comparado a los rayos X. El enrolamiento de los participantes fue aleatorio y mediante una rigurosa metodología se obtuvieron las imágenes por ultrasonido de contraste (CEUS) y rayos X (Rx). obteniendo 58 pacientes entre 2 a 5 años. 26 hombres y 32 mujeres, en su mayoría con una vía de abordaje yugular derecha (53.4%) y ventana subcostal (72.4%) con un tipo de CVC de tres lúmenes (89.7%), con un diámetro de 4.5 Fr. (55.2%). La media de IMC fue 20.22 kg/m2. La media del volumen de contraste fue de 3.5mL. (DE=0.76). Mediante agrupación binaria y regresión logística se logró determinar que, el mejor punto de corte corresponde entre los 300 a 500 milisegundos, ofreciendo, S:100%, E: 95,7%, VPP: 95,7%, VPN: 100% LR positivo: 23,4 LR negativo: 0,000 Exactitud 96,5%. Además, controlando el efecto de las co-variables el modelo de predicción, mejora su coeficiente de determinación de 87,1 a 91,1%. Se concluye que el método CEUS tiene cifras que validan su empleo con valores similares a la prueba de oro, con nulos efectos adversos y evitando la radiaciónen_US
dc.description.abstractIn the perioperative period, it is important to have a method that allows us to discern whether the tip of the central venous catheter is in an ideal position or not. Microbubble contrast ultrasound allows this, however, research is just booming in pediatrics. A diagnostic test study was carried out in order to determine the validity of the method compared to X-rays.
