Validación del ácido úrico como predictor de severidad en gestantes con trastornos hipertensivos del Hospital Vicente Corral Moscoso. Cuenca, 2020

Abstract

Background: Uric acid (UA) is a product of the degradation of nucleotide compounds and its elevation has been related to severity criteria in hypertensive disorders (HT) in pregnancy. Objective: to evaluate uric acid as a predictor of severity in pregnant women with hypertensive disorders at the Vicente Corral Moscoso Hospital. Cuenca, period 2020. Methodology: diagnostic test validation study. 190 clinical records of pregnant women with LT were reviewed. The value of the UA was contrasted with the clinical and laboratory diagnosis based on the severity of the TH. A form was designed to obtain the data. To measure association, the Odds Ratio (OR) and 95% Confidence Interval (CI95%) and chi-square with p value were used. The predictive capacity was determined with sensitivity (S), specificity (E), positive predictive value (PPV) and negative predictive value (NPV) at different cut-off points of the UA serum level. Results: the frequency of severe HT was 62.1%. UA ≥ 4.1mg/dl had a significant association with severe HT (adjusted OR = 22,186; 95% CI = 8.61 – 57.18). The best cut-off point for serum UA was ≥ 4.1 mg/dl (S = 92.4%, E = 54.2%). UA was considered to have good diagnostic capacity (Area under the curve = 0.75 – 0.90 (p = 0.000). Conclusions: uric acid levels ≥ 4.1mg/dl are associated with severity of hypertensive disorders, and useful as a predictor of severity.
